Plan ahead! We encourage you to make your final arrangement decisions in advanced and without stress. Make your wishes known and we will keep them on file at the church, even down to the Hymns you want. You may download and print a .pdf version of the Advanced Arrangement Form or pick one up at the office.

One of the forms, called the the Appointment of Agent to Control Disposition, must be notarized and name the person with the authority to decide the disposition of your remains upon your death (such as your spouse or child). It does not matter which funeral home you will use.
