Advanced Arrangement Forms

You may download and print a pdf version of the Advanced Arrangement Form. This form, called Appointment of Agent to Control Disposition, must be notarized and gives the named person the authority to decide the disposition of your remains upon your death.  It does not matter which funeral home you will use.
