Photo Permission

I give permission for my child’s photograph, without their name, to be used on the First Presbyterian Church of Granbury website, Facebook page, and other church-approved social media.

Permission to Participate and Release of Liability

I give permission for my above-named child to participate in activities at First Presbyterian Church of Granbury. I consent and agree to hold harmless First Presbyterian Church of Granbury, its agents, employees, and volunteer assistants from claims arising from my child’s participation in church activities. For Summer VBS, this permission includes participation in the bounce house and water slide.

In case of an emergency, I authorize First Presbyterian Church of Granbury and its designated representatives to obtain whatever emergency care is necessary for my child’s safety. I understand that I am responsible for payment of any medical or emergency services provided.
